I've played the entire game on nightmare. It's just been across multiple versions of the Act 3 beta.

For example, I got Yeldin and Jariel's quest achievements in the same run I killed the Kraken today (0.8.36), but no Kraken achievement? I can try loading a game this evening to see if it shows up this time, but that's odd.

Do you still get the boss achievements if they die to dots?

Yes I was unsure if to limit everything, but probably makes sense. I thought the bombs were the most powerful ones ?
Yes, the heavy bombs are the most powerful ones for fast dmg (unless the enemies dodge, of course). However, in some fights the other potions are equally "damaging" In case of Deceit, for example, where you can have 2 paralyzed thieves in your group and he is the sole enemy, paralyzed fighters flinging confusion, slow and scared potions at him seems rather unfair, because they can eff up his attacks following the paralysis. :P When it comes to manticore poison, I like the idea that it could be used, because it's a legendary item, but lorewise it makes absolutely no sense to be able to use it because of the "deft fingers"-talk upon receiving it. A paralyzed fighter handling it sounds more like risk to sprinkle it over your own team. :P I'd say just deny all the offensive stuff and keep the haste/health/mana/resurrection potions and scrolls available. I think it's amazing enough that manticore can be used on Bosses and their whole team when not paralyzed. Also, the world is full of games where you can't even use healing items when paralyzed, nor move.

Yes doesn't matter how it dies, since I assign the achievement at end of combat. The fact that you played using multiple versions of act3 COULD be the issue, but is still strange. Waiting to see what other people say, but now I have a doubt - you didn't fight the Kraken in the Sunken Ship quest right? because that's when you get the achievement :) he is one of a tough guy to die (you have several fights against him!).

Yes that's what I'll do then :)

Can I farm these blobs forever? Take straight tunnel = fight, every time it seems.

(but these are freaking long fights so I've probably only done 3-4)

Goddamn how do I even stop fighting them and get back to the boat? :P Never mind I finally managed to do that. That cavern system needs a map.

Yes I didn't put a time limit, because I thought that since you can't rest unlimited times (I think you can rest only once) people won't be able to do much grinding :)

So if I hated myself enough I could grind level cap there.

This is not a new bug (if it even is a bug), I've just forgotten to ask about it again and again...

In Act 1, when the twins go to the tavern to talk with Daronas about the graveyard, they are all stonefaced through the whole convo. No expression changes on any of the three sprites, even though some of their lines indicate surprise, etc. Is this on purpose or a bug?

Is because they're SHOOOOOOOCKED! Honestly I am not going to fix such problems in an insanely huge game like this, otherwise I would never finish it

Very likely your last save was from a very old version (before I used the new Ren'Py framework).
